Communication Log for Students, Parents, and Families

Developing and maintaining a record of your contacts is important in establishing a working relationship.  Keep a lot of your collaboration with students, parents, and families for one trimester of your choice.  Identify the form of collaboration, the date of contact, and a brief explanation of the reason for the contact.

Select one time period:

  • Trimester 1:  August - October
  • Trimester 2:  November - January
  • Trimester 3:  February - May

This log must reflect ongoing communication during the period.  It must provide examples of a variety of resources or tools to communicate (ie: technology devices, interpreters, etc.)  Forms of collaboration may include but are not limited to:  meetings with parents such as conferences, IEPs, Student Study Teams (SSt); phone/ mail conversations, newsletters, notes, etc.  The first three communications listed are critical and are required items.
